...by Focusing on what REALLY makes a Difference to your Scores.
If you want to get your fellowship with RACGP, then this is maybe the most important page you read.
Preparing for these exams is probably the last exams you will ever take and approaching this is a lot like climbing a large and intimidating mountain.
Yes your goal is to actually get to the top and get your fellowship by passing your exams but most of the important work is in your preparation, training and equipment - just like climbing the mountain.
In the last 5 years, a lot more people are taking the exams and the standard has got higher. We've learned to a lot about the best approach to getting you to the top.
If you want to utilize what's been learned, keep reading now!
This journey will take work, there is no getting around that fact.
If you are not prepared to put in many hours preparing for these exams then you should stop now and do something else.
This is not for you.
Complaining about the exams will not get you through them.
The exams are challenging and difficult and this is not going to change.
The RACGP wants the standard of GP to remain high in Australia and they are the gatekeepers to who gets to call themselves a fully qualified GP in Australia.
If you want to join them, and it can certainly be done then read on.
So let’s take a moment to look at some of the traps that exist on this path, into which many previous candidates have fallen into…
Rote learning or taking endless practice exam questions will not prepare you these exams.
The College knows that the past questions are out there in the internet even though they have expressly said this is against their “policy”.
Here’s what people don’t think about when they start rote learning past questions...
The methodology of rote learning and practicing does not provide the flexibility you need to adapt to specific questions that are asked to you on the day of the exam.
Small changes in the question - even if it looks just like that one that you may have seen before - change the correct answers for that new question.
So…
You need to have a way to respond to the specific nature of that particular question.
It’s not to say that practicing questions is unhelpful, this is not what I am saying…
You need to be practicing the underlying thinking that goes into working out what are the correct answers…
This is quite different to rote learning questions and answers…
Instead you will be practiced and ready to adapt to the specific stem provided in your questions.
You need to practice doing this methodology so you are prepared and ready when you sit down to take your exams.
Look, all the promises in the world can be made, but at the end of the day, you are the one who has to sit down in the AKT exam and answer the questions.
You are the one who has to sit down at the KFP and answer the questions.
You are the one that has to show up to the OSCE and take the cases that are present
You must be willing to take full ownership for your outcomes.
Nobody else but you can take your exams and so whatever your outcomes are yours.
You can take my course, other courses : but you must know that you will get out what you put in.
Yes, there are massive benefits from leveraging work already done, learning better ways to think in GP cases and benefit from a community of people working towards the same goal…
But if you don’t put in the work to actually use the resources, no amount of courses will help.
You can definitely prepare and do really well in these exams but there is no guarantee...
No one but yourself will be able to step-up and go "Yes! I can actually do this."
No one but yourself will know whether you have the discipline to actually put in the hours each week it takes to go through a course.
Know this though… If you have put in the hours in a smart and leveraged way, increased your clinical knowledge, raised your clinical skills and mastered clinical reasoning then you will have a confidence in your bones that will show up in your results.
So at the end of the day - you are responsible 100% for your outcomes.
If you would like to step into a leveraged and smart methodology of preparing for your fellowship exams, then there is something that you should know about...
That is the General Practice Exam Accelerator.
When you join of General Practice Exam Accelerator, there are number of things that you gain that will help you in your journey up the mountain of getting your FRACGP.
This course is designed to help you overcome the major challenges of modern Australia GP exams.
There is too much information.
The course contains an 8 week structured roll out of material that are already cut down to the essentials...
They cover the 20 major categories of GP (Cardiology, Respiratory, Urology, Gastroenterology etc) and give you either one page diagnosis and management checklists or more extensive legacy mindmaps.
These Legacy Mindmaps are more detailed than the checklists and can be used as a stepping stone to go from Checklist to Mindmap to Source text allowing you to incremently build greater depth in your understanding and knowledge.
This is designed to give you the guts of the large number of medical topics so you can rapidly cover the content and focus on the key features of each topic.
You can go through these key feature notes and then expand out into further reading and research on these topics.
Essentially you are getting a solid foundation for a topic which you can then go and fill in more detail without having to wade through and spend 2-3 hours creating notes.
Remember there are 400+ topics that could show up in this exam so just to get through the clinical note making could take you 800+ hours.
The Key Feature Checklists are extremely helpful for giving the clarity that you need that when you are working through AKT questions around diagnosis and management.
Using the Key Feature Checklists you will have the key features of diseases fresh and ready for you to apply to the case.
In the KFP you will often be required to choose between 2 or 3 possible diagnoses off just a couple of symptoms and signs.
If you are clear on exactly what each disease has as it key features then it is going to be a lot easier than having a vast amount of information that is not properly structured and then trying to remember - what was the most likely disease to have hemoptysis, chest pain and fever…
It sounds easy when you are just reading through this page, but add in exam stress, expectations and all of the other information you have learnt in preparation and suddenly it can be a LOT harder to pull out the correct diagnosis.
That’s why having the Key Feature Checklists & Legacy Mindmaps as a key part of revision strategy really makes sense.
You can practice KFP thinking beforehand!
This was a major insight that I had AFTER having taken my KFP.
Oh… I could have prepared working through from presenting complaint and thinking about what key questions separate out the differentials possible from that presenting complaint.
As part of the General Practice Exam Accelerator is a number of “pathways” that take you from presenting complaint through to the diagnosis and the key factors that determined what actually is going on.
The pathways mean that that you are already thinking about what differentiating factors there are from a differential…
For instance, let's say the case is about vertigo.
You might want to think about whether it acute or chronic because the causes of acute vertigo and chronic vertigo are different.
And then you might think about what kind of questions and examination findings are going to be able to allow you to work out what’s happening…
Let's say that their hallpike maneuver is positive and they have had similar episodes of vertigo before and the actual duration of the vertigo is very short… well hopefully that disease picture is bringing Benign Positional Paroxysmal vertigo as the diagnosis...
BUT…
If there was a preceding flu like illness, hearing loss and the episodes last for minutes to hours then you would be thinking… ahhh yes….
That sounds like viral neuronitis.
So having these pathways with the key differentiating factors is going to be key to being best able to answer the KFP type of questions.
When you join the General Practice Exam Accelerator, you receive two fortnightly homework cases that work through KFP style questions.
You take them on Monday and then on Thursday there's a live webinar with myself or one of our medical educators..
On this webinar, we review the answers and learn best practices in KFP.
It's okay to make silly mistakes because this is the time and place to learn and get your KFP answers up to scratch.
So it really gives you the practice for how to think and how to get through the KFP.
It's like training for the mountain climb - getting the practice in, getting used to thinking this way, getting ready for sitting down at your exams and doing really well in your exams.
You also join the General Practice Accelerator’s private Facebook group.
This is to help you with support from other members of our course.
You can share learnings and ask for help.
You can share your fears and find out that others are experiencing similar feelings.
Remember that the exam is not based on certain number getting though but rather it is based on meeting a set standard…
If this was the exam setup then there would be no point in sharing information.
Since the exams are about meeting a set standard, then moving to a cooperative model of helping each other is a smart move.
Having access to the other doctors that are sitting on this exam is basically like having your own team to provide additional support, clarification and encouragement as you prepare.
The exams are not just to show that you are safe, competent and skilled as a GP, but also they represent the opportunity to set you up for a great life as a General Practitioner.
As you pass these exams you will know...
You are confident in your knowledge and skills.
You go home at the end of each day knowing that you've helped another 20+ patients and you've made a big difference in their lives.
You have your patients complain that they can't get in to see you because you are so heavily booked and this definitely a problem that you want to have.
